Chronic Disease Management

The doctors and nurses at Wangaratta Medical Centre work together to manage patients with chronic diseases such as asthma, coronary heart disease, chronic obstructive pulmonary disease, diabetes and renal impairment.

Patients with a chronic disease are often eligible for a care plan giving them access to subsidised allied health services. We encourage all patients with a chronic illness to discuss a chronic health care plan with their GP.
